When our Information Department explains the Truehope program to new participants and verifies their personal information they also confirm the appropriate diagnosis. By doing this, the appropriate Symptom Evaluation Form (SEF) is immediately attached to each participant’s file. Individuals that utilize this helpful tool and consistently fill out these symptom forms are those that generally see the greatest success with the Truehope program.
The goal of our support staff is to help you effectively attain the most complete response from EMPowerplus you can. And the information you provide through the SEF is invaluable to the Truehope staff in supporting you on your journey to mental and physical wellness. From this information, our support staff can more accurately determine when drug reductions are necessary and understand better how you are responding to critical transitions along your road to recovery.
The SEF can also help you track your own progress. Those who have taken our suggestion, to regularly fill out their SEF, have enjoyed monitoring their own symptoms much more accurately. Instead of basing important health decisions just on how they are feeling from one day to the next, they can look back over months of progression and literally “see” how far they have come. Many participants have told us that filling out the SEF's daily helps to keep them on track and not lose sight of the attainable goal of achieving wellness using EMPowerplus.
So if you are fully committed to eliminate mental illness from your life, make the SEF a part of your daily routine. You cannot afford to pass by any tool that could greatly enhance your ability to take control of your illness. Daily filling out your SEF will help you realize why hundreds of others before you, who now enjoy the gift of mental wellness, didn't give up—no matter how many bumps slowed their journey.

- SEF's are available on mytruehope.net—our secure participant resource website—which you can access from the comfort of your own home, as long as you have a username and password.

A number of people have asked Truehope to provide a SEF for children who suffer with bipolar disorder, as the adult SEF for bipolar is not sufficient for children. We would like to invite any parents whose children suffer or have suffered with bipolar to forward us their suggestions as we develop this new SEF. Please forward your input to support@truehope.com. We eagerly await your input and hope to have this SEF available soon. To those who have already sent in their suggestions, we greatly appreciate your interest in helping others.
